xt926 stuck in AP Flashboot mode

Phone is a Motorola Droid Razr Maxx HD

 

xt926 stuck in ap flashboot mode.

I know the attached photo is not readable so I entered it:

 

AP Flasboot Flash Mode (S)

10.9B(*) (sha-61146a2, 2014-05-02 03:15:48)

eMMC Info: Size 32GB

 

Device is LOCKED. Status Code: 0

 

Battery OK

 

Transfer Mode:

USB Connected

 

No valid PIV block in SP for system

PIV validation failed (system)

Fastboot Reason: Boot Failure

 

Going into recovery with (Vol + and Pwr) works.

However, the phone immediately flashes the red Motorola logo then right back into flashboot mode.

 

The phone is stuck in fastboot mode.

I've tried to reinstall stock rom via RSD Lite. Will not work. Starts, then fails within a second.

here is the fix 

 

1. adb reboot bootloader
2.fastboot oem fb_mode_set
3.fastboot oem fb_mode_clear
3.fastboot erase userdata
4.fastboot erase cache
5.fastboot reboot

Where do I enter these commands?

you need to connect your phone to the computer 

then open dos window then add the commands
