Could be a few different reasons.... are you using the OEM cable that came with your phone? .... did you have plenty of charge prior to starting the utility?.... try using a different USB port on your computer.... reboot your computer prior to running the utility..... you can also try to get these 2 files here and replace the ones in the "files" folder....(updated fastboot.exe and adbwinapi.dll) ... then run the utility.... just remember your phone will not charge while in AP Fastboot mode so use caution.....AP fastboot flash at the top and at the bottom it says fastboot reason: flash failure. Any help would be greatly appreciated.

Hmmmm.... with a flash failure, (basically a half programed phone) it may not recocognize the presence of the sim card anyway (because the software is not there to tell it) .... if you still have enough charge, try using the files I included (above post) and run the utility again......the problem i'm having is that the phone is stuck in ap fastboot mode and i can't get it out, even when i can get it to appear to do a factory reset it tells me there is no sim card installed.

Copy and paste into the files folder (where you extracted the utility) on your computer then try running the utility again....Forgive me i'm new to this, how do i use the files you suggested, what do i do with them since my phone won't do anything, can't even get my computer to see the sd card.

1.) Boot phone into "AP fastboot" mode.
2.) Run the utility
3.) Select option 1 from the menu
4.) That's it - I'll be back to stock (erasing data on phone)
Am I missing anything? If I am, what is it?
Thanks in advance for the feedback, and as always, thanks to you developers for keeping me on the cutting edge!

Alright. Given that Verizon has made the announcement for the official OTA, I want to revert back to ICS from the leaked (unrooted) JB by using Matt's tool (sounds wrong, but you get the drift). Anyhow... I'm a stickler for doing things right the first time and like to have the step by step instructions down pat. That being said am I correct in saying the steps are:
1.) Boot phone into "AP fastboot" mode.
2.) Run the utility
3.) Select option 1 from the menu
4.) That's it - I'll be back to stock (erasing data on phone)
Am I missing anything? If I am, what is it?

Yes that is it, I know it is implied, but make sure to have the correct drivers installed on your PC and have it connected via USB cable.
