Having an issue I haven't seen before. I went into the safe strap 3.11 recovery to wipe my internal storage and in doing so, the app displays a Successful but when you look at the terminal screen you see the following:

Using rm -rf on '/dev/block/mmcblk1p25'
E:tw_format_rmfr: unable to locate volume for device "

And it don't wipe the internal storage. I had to go into the stock ROM and then connect the phone to my computer and manually delete the files through the computer.

Anyone else having this type of issue?

If you go into SS and mount, does it show that it is mounted?  - be sure that you are looking at the internal SD not the external ( I am sure you probably did that ) If it is unmounted and you try to mount it, does it give you an error?

Another thing is that Safestrap may be set up like that purposely so Noobs don't accidentally wipe their internal Sd and then get their panties in a bunch over it. Just guessing here...

I'm having a similar issue as the OP, but have not been able to figure out a solution. The Internal SDCard will not mount, i cannot select it in the SS mount menu (box remains unchecked), nor is it visible while plugged into either linux or windows systems (with moto drivers installed prior to this issue).

The problem only happened after I flashed the 4.1.2 update via RSD Lite, then followed the root how to from djrbliss

I tried one solution I found, where someone said to use ROM Manager to Fix Permissions, but that didn't work like it did on his bionic. I got a "An error occurred while attempting to run privileged commands!" message that didn't give me an option to format like it did for him.

Any ideas?
